Most initial appointments are scheduled within one to three weeks of having contacted the office.
Initial phone appointments are scheduled to determine your needs and to clarify how Dr. Upton can be of assistance. If you decide we might be able to work together, an initial assessment will be scheduled.

If using insurance, you are responsible for providing the information needed to bill your insurance company on the appropriate forms. If Dr Upton is out of network with your insurance, you must submit your statements for Out of Network reimbursement.
